Complex Chronic Disease Team
The aim of the Complex Chronic Disease Team (CCDT) is to provide a range of services for people with complex health issues who are at risk of admission or readmission to hospital or frequent presentation to Emergency Departments. CCDT provides assessment and intervention to support people in managing their complex care needs and chronic disease to minimise complications.
Provides geriatric medicine consultation service at two community health sites (North Lakes and Nundah) and home visits by geriatrician, geriatric medicine registrar and GP with special interest (GPwSI) in older persons’ health.
CCDT also provides multidisciplinary clinics including Dietician, Exercise Physiologist, Neuropsychologist, Occupational Therapist, Physiotherapist, Psychologist, Social Work, Speech Pathologist.
