Nurse Navigator – Older Persons
- Monitor high needs older persons, identify actions required to manage their health care and direct patients to the right service, at the right time and in the right place
- Provide an end-to-end care and coordination service along a patient’s entire health care journey
- Educate and help patients to better understand their health conditions and enable them to self-manage or participate in decisions about their health care. In some cases, this may even reduce the need for hospital admission.

SAGE Sub Acute Geriatric Evaluation Unit
SAGE is a specialised (11 interim care beds and 16 geriatric evaluation beds) unit committed to encouraging the older person to maintain function and promote qualify of life through a goal therapy multidisciplinary program with the aim to discharge to their destination of choice.
The eligibility criteria include:
- Over the age of 65 or 55 for indigenous people
- Younger onset dementia
- Geriatric syndromes
- Requiring specialised geriatric assessment
- Complex discharge planning
- And / or requiring nursing home placement
SAGE is located on the 5th floorof the new hospital building. The service offers a total of 27 inpatient beds. SAGE also services North Rockhampton and Eventide nursing homes.

Geriatric and Rehabilitation Services (GARS)
- GARS is a specialised unit committed to restoring and maintaining the optimum level of function of the individual and significant others, enabling them to achieve their lifestyle goals.
- GARS is located on the Rockhampton Hospital Campus in a purpose built facility. The service comprises of a 16-bed inpatient service as well as 15 bed capacity for day hospital rehabilitation and outreach service.
- GARS also provide a rehabilitation outreach service to Mt Morgan, Capricorn Coast and Gladstone.
- The Living with Amputation Service is also provided at GARS.

Older Persons Mental Health Service (OMPH)
The purpose of the Older Persons Mental Health Acute 8 bed inpatient unit is to provide acute care and management for people suffering from symptoms of an acute deterioration in their mental health, or exacerbation of mental illnesses who require a period of inpatient admissions to stabilise their health
Community Assessment Team
OPMHS is aimed primarily at people aged over 65 and over 55 years for indigenous people with:
- Mental illness complicating an age-related disorder
- Mental illness which has arisen for the first time
- Age-related disorder complicating a pre-existing mental illness
- Younger people who are affected by age related neurodegenerative problems such as Alzheimer’s disease
- Older persons with a mental illness who are otherwise fit and well should receive treatment from general adult mental health services

Residential Acute Care Service (RACS)
- Residential Acute Care Service provides a clinical consultancy service to the 10 Residential Aged Care Facilities (RACF) within Rockhampton and Gracemere areas.
- The service provides a timely triage assessment and treatment plan service to the residents with an acute illness in the RACF in collaboration with the residents’ GP, RACF staff and RACS staff. The clinical governance regarding the clinical treatment remains with the resident’s GP.
The aim of RACS is to:
- Provide a safe and appropriate alternative to hospitalisation
- Reduce the need for residents to leave the RACFs to attend Emergency Department or be admi8tted to an acute hospital for conditions that may be safely treated within the RACF

Community Palliative Care
Community Palliative Care provides a palliative care consultation and early intervention service for the end of life clients with a chronic illness to enable the option of palliative care in a partnership model with other community service providers.
Supports client and their families facing the challenge associated with a life-threatening illness, prevent and relieve suffering through assessment and care planning in a holistic manner with the approach to improve quality of life and end of life experience
