West Moreton’s Heart Failure Service has implemented a new method of service delivery. The West Moreton Health / Philips Heart Health Hub is a pilot program using remote patient monitoring and video conferencing to virtually manage heart failure patients, specifically focusing on timely medication titration in their own home.

This presentation will assess the feasibility and acceptability of Heart Failure Service virtual model for medication titration for heart failure patients, which is facilitated by the heart failure nurse practitioner and cardiology specialist. It will show how the new streamlined service means staff can perform titration without disrupting the patient or requiring them to come into hospital, resulting in significant health system savings as well as reducing barriers to accessing care and engaging with clinicians – regardless of where a patient lives.
