TeleHandover patient centred approach for clinical handover of patients transferring between facilities

TeleHandover delivers team-to-team clinical handover for patients transferring between hospitals or to another healthcare service or facility via videoconferencing.

Proudly supported by Clinical Excellence Queensland, this new handover model takes on the case conference format and includes the patient, family and carers, and in interdisciplinary teams from both facilities. This enables timely, shared clinical decisions to be made with consumer involvement and facilitates the safe transfer of care of patients to the setting most appropriate to their needs, often closer to home and their support base.
