Background
The Healthcare Improvement Unit (HIU) developed an Inter-Hospital Transfer Form in 2015 (as the then Clinical Access and Redesign Unit).
While it was predominantly intended to be used as an administrative form, it came to be used as a clinical handover tool in a number of sites.
It had several problems including it being labour-intensive, as it is has very limited auto-population and transmitting this information via email is in contravention of legislative requirements.
