An Inter-Hospital Transfer application will improve statewide access and management of Inter-Hospital Transfers for metro, rural and regional communities. The objective of this project supports the 10 year vision My health Queensland’s future - Advancing health 2026: Delivering healthcare, Connecting healthcare, and Pursuing innovation.

To develop and implement a State-wide Inter-Hospital Transfer Application.


  • Improved patient recognition, registration and auto-population of demographic details.
  • Ability to modify the Inter-Hospital Transfer form to meet clinical and administrative requirements.
  • Meet Queensland Health legislative requirements regarding the transfer of confidential information between sites.
  • Improve collection of data relating to Inter-Hospital Transfer utilisation and the development of reporting capabilities.


The Healthcare Improvement Unit (HIU) developed an Inter-Hospital Transfer Form in 2015 (as the then Clinical Access and Redesign Unit).

While it was predominantly intended to be used as an administrative form, it came to be used as a clinical handover tool in a number of sites.

It had several problems including it being labour-intensive, as it is has very limited auto-population and transmitting this information via email is in contravention of legislative requirements.
